Information Collection and Use
PCRB/DCRB are the sole owners of the information collected on this site. We will not sell, share, or rent this information to others in ways different from what is disclosed in this statement.
PCRB/DCRB collect personally identifiable information (information not otherwise available for public viewing), when you register for PCRB/DCRB accounts or when you complete and/or submit Financial Data Manager (FDM) Application information. This information is used to contact you, the member, about changes to the service and to provide a method for the PCRB/DCRB or the Insurance Department to contact the member for communications regarding the Financial Data Manager (FDM) Application or the Financial Data Incentive Program.
PCRB/DCRB also automatically records information on our server logs detailing user activity.
Registration
In order to access members-only features of this website, users must complete a registration form. During registration a user is required to give their contact information such as the name of a main contact person for their company, e-mail address of the contact person, address, phone number and User ID and Password. A Password will either be assigned by the PCRB/DCRB or by the Application.
Sharing
Personally identifiable or sensitive information provided to the PCRB/DCRB at this site is not disclosed to third parties. However, from time to time, the PCRB/DCRB may be required by law to disclose personally identifiable, sensitive, non-public financial and insurance claims information to third parties in response to subpoenas, court orders, legal process or in response to the requests of the State Insurance Regulator. In the event that disclosure of such information is required, the member will be notified of the disclosure within a reasonable timeframe, and PCRB/DCRB will disclose to the party or parties receiving the information that the member has been notified of the disclosure.
The functions of the PCRB/DCRB include compiling financial data, classifying employers and proposing underwriting rules, policy forms, loss cost values and rating plans for workers compensation within their states. In order to carry out this function, aggregated insurance and financial data is compiled into various categories and reports. We will share that aggregated insurance and financial data with the State Insurance Regulator, our members, and the public. This aggregated information is otherwise not linked to any personally identifiable, sensitive information, except where expressly requested by the State Insurance Regulator.
Cookies
A cookie is a small text file stored on the user’s hard drive containing non-personal information about the user that the website uses to recognize repeat visitors, facilitate the user’s ongoing access to and use of the site and allow a site to track usage behavior and compile aggregate data that will allow content improvements. Usage of a cookie is in no way linked to any personally identifiable information while on our site. Once the user closes the browser, the cookie simply terminates. Cookies are not programs that come onto a user’s system and damage files. Cookies work by assigning a unique number to the user that has no meaning outside the assigning site. You will be required to enter your private User ID and Password each time you access various Bureau applications. You should make sure that you do not share this information with others.
Correction/Updating Personal Information
If a user's personally identifiable contact information changes, if a member no longer desires our service, or if a member would like to disable a user, we will endeavor to provide a way to correct, update or remove that user's personal data provided to us.
Requests to change or update personal information or to change or disable a user are handled differently for each specific Application. Changes to the Users information can be made via the Application Login Member’s Portal or by sending an e-mail detailing your request to [email protected] for help.
Notification of Changes
If we change our privacy policy, we will post those changes on our website so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we will disclose it. If at any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify our users in advance. Users will have a choice as to whether or not we use their information in this different manner. Otherwise, we will use information only in accordance with the privacy policy under which the information was collected.
